About Tempe Laser Center

Tempe Laser Center is a laser hair removal and aesthetic clinic located in Phoenix, Arizona. The clinic offers laser hair removal across all Fitzpatrick skin types, photo facial treatments, and adjunct aesthetic services.

The clinic is operated by the founding team of Formfy. It is used as the operator-dogfooding reference site for the Formfy platform — every product change is run through real client appointments at Tempe Laser Center before it ships to other customers. A sister clinic, Westwood Laser Spa in Los Angeles, runs the same Formfy workflows.

How long does it take to convert an existing PDF form?

Most forms convert in seconds. Formfy scans the document, identifies every fillable field, and overlays live inputs automatically. Edge cases — multi-column legal templates, hand-drawn fields — may need a few seconds of manual cleanup.

Do clients need to install an app?

No. Clients open the form in their phone's browser from an SMS or email link. No download, no account, no friction.

How does Formfy handle data security and client privacy?

Forms and signatures are transmitted over TLS 1.3, stored encrypted at rest, and isolated per-account. SOC 2 Type II audit is in progress with a target completion in 2026. Formfy follows data minimization principles consistent with CCPA and GDPR, including the right to deletion on request. For clinics that need a signed Business Associate Agreement today, Formfy is not the right fit yet — that infrastructure is on the roadmap but not shipped.

What happens if a client doesn't have a smartphone?

The form also works on desktop browsers via email link. If a client arrives without completing it, your team can hand them a tablet or accept the PDF as backup.

Can I customize the form after Formfy converts it?

Yes. After AI overlay, you can edit field types, add fields, change validation rules, and adjust the SMS copy that goes out with the link.

How is the e-signature legally valid?

E-signatures captured through Formfy meet the requirements of the ESIGN Act (US, 2000) and the Uniform Electronic Transactions Act (UETA) adopted by 49 states. Each signature is bound to the form by a cryptographic timestamp, IP address, and user-agent fingerprint. For industry-specific consent requirements (state cosmetic medical practice rules, state-licensed tattoo artist consent forms, etc.) consult your own counsel.

Does Formfy integrate with my scheduling or CRM software?
